Assisted Living Waiver Program (ALWP) Eligibility
We are a provider of the Assisted Living Waiver Program for both Assisted Living and Memory Care. If you are looking for placement, please contact us for current availability. If you are just learning about the program, we are happy to help guide you to the proper resources to get that process started.
Requirements:
Must have Medi-Cal with NO share of cost. ( monthly income is typically under $1400.00 a month) for placement.
Have a medical need that Assisted Living options would be able to help with.
Willing to have placement in Sacramento County ( and willingness to switch Medi-Cal to Sacramento County if it is assigned to another county in California)
If there is a diagnosis of Mild Cognitive Impairment (MCI) or Dementia- there must be a Power of Attorney (POA) of record.
